Client Referral Packet

Thank you for your interest in 2nd Chance Solutions, Inc. 

Our mission is to provide safe, affordable, and supportive housing for adults seeking stability and greater independence.

We strive to create a respectful environment where residents can focus on achieving personal goals while living in a clean, secure home.

We appreciate the opportunity to partner with social workers, case managers, hospitals, behavioral health providers, community agencies, and other referral sources.

Admission Criteria

Applicants should:

✓ Be able to live independently.

✓ Be willing to follow house rules.

✓ Demonstrate respectful behavior toward others.

✓ Have a source of income or an established financial plan (if required).

Each referral is reviewed individually.

Referral Process:

Referrals can be submitted by:

  • Social Workers

  • Case Managers

  • Hospitals

  • Behavioral Health Providers

  • Community Organizations

  • Churches

  • Probation or Parole Officers (if applicable)

  • Self-referrals

To begin the referral process, please provide:

  • Referral Form (use the link above)

  • Brief client history

  • Current medications (if applicable)

  • Emergency contact

  • Income verification (if applicable)

  • Identification
